Heaven On Earth is a healing community where people are seen, supported, and given space to thrive. We call our residents Stars because every person shines with possibility. With the steady care of our Earth Angels—our trained, compassionate caregivers—we create the structure and warmth that make real change possible.
Our Mission
To provide a safe, stable, and nurturing environment that restores dignity and builds independence. We do this through Comprehensive Case Management, Outpatient Care, Wraparound Services, and daily enrichment activities tailored to each Star’s needs.
What We Provide
- Housing & Daily Living – Safe, supportive homes with clear routines and accountability.
- Nutrition – Three balanced meals every day, centered on wellness and comfort.
- Transportation – Reliable rides to appointments, therapies, work, and essential errands.
- Case Management – Personalized plans, advocacy, and navigation of benefits and resources.
- Outpatient Care – Therapy, recovery support, and wellness services delivered in the community.
- Wraparound Support – Coordinated services that connect health, home, and hope.
- Activities & Enrichment – Stars can sign up for structured activities through our partner centers, offered on a block schedule, with plans for an on-site Activity Center in the near future.

Earth Angels are the steady hands and kind hearts of our program, professionals who combine practical support with genuine respect. They coordinate services, encourage healthy routines, and champion each Star’s goals so progress feels possible and sustainable.
How We Work
Every Star begins with a personalized consultation. Together, we identify priorities, set goals, and build a care plan that aligns housing, health, activities, and day-to-day support. We review progress regularly and adjust services as needs change.
Who We Serve
People transitioning from homelessness or hospitalization, individuals in recovery, returning citizens, veterans, older adults, families, and single parents—anyone who would benefit from stable housing, coordinated care, and compassionate guidance.
